Initial Comments:
A letter of request was received in this office on November 6, 2023, amended and addended on December 5, 2023, and processed on December 5, 2023, for a bed relocation and bed capacity decrease to be effective December 1, 2023. The bed relocation was identified along with the certified beds being decreased from six beds to five beds as outlined in the bed complement below:
Bedroom #1 will relocate 1 bed to Bedroom #4 which reduces the capacity in Bedroom 1 from 2 beds to 1 bed, and increases the capacity in Bedroom #4 from 1 bed to 2 beds.
Bedroom 2 will reduce its bed capacity from 2 beds to 1 bed.
Bedroom #3 will remain unchanged with a capacity of 1 bed.
Merakey Allegheny Valley School Green Garden remained in compliance with the requirements of 42 CFR Part 483, Subpart I Regulations for Intermediate Care Facilities.
